How to Update Your Name & Address
Please download, complete, and return the Change or Withdrawal Notice Form either by email, mail, or fax. If you do not have the ability to download or print the document, call the ACP Office to request a physical version be sent to you.
Forms submitted without the participant’s secret word and signature will be rejected. Please call the ACP Office if you forget your secret word.
How to Renew or Reactivate Your Membership
Please download, complete, and return the ACP Application either by email, mail, or fax. If you do not have the ability to download or print the document, call the ACP Office to request a physical version be sent to you.
Include your current or former ACP ID number on the Form and be sure to add any family members you need to keep part of your household.

How to Withdraw from the Program

What do I do if I get cancelled from the Program?
The ACP Statute states that ACP participants may be canceled from the program under the following circumstances:
- The participant does not notify the ACP they have changed their name within 14 days
- The participant does not notify the ACP of a change in physical address within 14 days
- The participant’s mail comes back undeliverable, and the participant does not provide a new one within 14 days
- The participant provided false information on their Application
- The participant’s membership term has expired, and the participant does not complete an Application to renew within 14 days
Participants may re-apply to the Program to reinstate their membership with the required updated information. However, under certain circumstances, participants that are cancelled will not be able to rejoin the Program, such as if they provide false information. Additionally, participants that move out of state will not be able to rejoin from another state.
What can the ACP Office help me with?
The ACP Office can help with the following things:
- Providing a participant verification letter for government agencies or private businesses that request one
- Providing a district verification letter for school districts
- Informing you the last date you received mail
- Providing instructions on how to change information with the Program
- Referrals to other State programs and services
What if I move out of state?
Fortunately, over 40 states have some version of an Address Confidentiality Program, and every neighboring state to New York has a program. The ACP team is happy to refer you to your new state’s Program and requirements.
Participants that move out-of-state are unable to stay in the New York State Program. The ACP will send a courtesy mailing of any held mail once the participant has moved.
