Overview

Each Hearing Aid Dispenser Business location that dispenses hearing aids must obtain a separate registration and employ at least one registered hearing aid dispenser at that location. It is the responsibility of licensees to understand the Hearing Aid Dispenser License Law.

Fees

There are fees associated a Hearing Aid Dispenser Business license. You may pay these fees by check or money order made payable to the Department of State or by MasterCard or Visa using a credit card authorization form. DO NOT SEND CASH. Application fees are nonrefundable. A $20 fee will be charged for any check returned by your bank.

  • $150 - Registration fee (10 or fewer employees)
  • $100 - Renewal Fee (10 or fewer employees)
  • $200 - Registration fee (more than 10 employees)
  • $150 - Renewal (more than 10 employees)
  • $25 - Notice of Employment Status
  • $10 - Change of Personal Name and/or Address (No fee is required if a name change is the result of a change in marital status)
  • $10 - Duplicate License/Registration Request

Renew or Update

Renew Your License

You are eligible to renew your license 90 days prior to your license expiration date. Your license expiration date is printed on the license.

Renewal forms are mailed approximately 90 days prior to the license expiration date. Complete and submit the renewal form to the address on the form.

If you are eligible to renew your license and you have not received a renewal form, email the Division of Licensing Services and request a form. Your email must include your full name, mailing address, license type, unique identification number and expiration date of your license.

Update Your License

To update your personal name, business or residence address, download, complete and submit the Change Notice to the address on the form.

To request a duplicate license certificate, download, complete and submit the Duplicate License/Registration Request to the address on the form.

FAQs

Who must apply for a Hearing Aid Business registration?

Registration is required for each permanent business location at which hearing aids are dispensed.

Business applications must be made by the business entity and signed by the sole proprietor if the business is owned and operated by an individual; by a trustee if the business is a trust; or by an officer or partner if the business is an association, corporation, partnership or limited liability company or limited liability partnership.

Each business location that dispenses hearing aids must obtain a separate registration and employ at least one registered hearing aid dispenser at that location.

What is a not-for-profit corporation?

This is a corporation formed under the Not-for-Profit Corporation Law that has filed a Certificate of Corporation with the Department of State, Corporations Division.

Are not-for-profit corporations exempt from the hearing aid business application fees?

No. They must file an application and submit the appropriate application fees.

Parking
Limited on street metered parking is available for 1 Commerce Plaza in Albany. The Office of General Services has various parking lots throughout downtown Albany which have limited visitor parking. To find out more information about the

New York City: This office is open for all services with the exception of walk-in apostille processing. Apostilles will not be processed without a previously scheduled priority appointment.
