Eligibility

To be eligible for the New York State Address Confidentiality Program, an applicant must:

  1. Be a New York State resident
  2. Fit in one of the two eligibility categories, or be a family/household member of someone in these categories:
    • A victim or survivor of domestic violence, stalking, sexual offenses, human trafficking, or kidnapping that has left their previous housing out of fear for their safety
    • A reproductive health care services employee, volunteer, or patient.

 

Frequently Asked Question: Do I have to share my order of protection or legal documents with the ACP to apply? 

No, the ACP Application does not require documentation of victimization. Applicants sign an affidavit attesting that the information they provide is accurate and they understand who the program is intended for. Any participant that provides false information on their application will be removed from the program.

There are several ways to apply to the New York State ACP. See below:

  • Online Application Portal
  • Download the Application in PDF form and either mail, email, or fax it to the ACP Office
  • Call the ACP Office and ask for a paper copy to be mailed to you with a return envelope.
  • Visit an Application Assistance Provider and ask for their help completing the application.

Basic Program Rules

To participate in the ACP, we require that you adhere to program rules and guidelines. The rules are outlined in the ACP Application and a participant’s welcome packet, but some basic rules include:

  • Keep us updated on contact information and mailing/ residential addresses
  • Only provide us with truthful information
  • Don’t send packages to the ACP PO Box – we can only forward packages that are prescription medication or clearly marked as being from a government agency.
