Part 1203 Review and Reporting Forms

Title 19 NYCRR Part 1203 requires every city, village, town, and county, charged with administration and enforcement of the NYS Uniform Fire Prevention and Building Code (Uniform Code) to annually submit a report of its activities to the Secretary of State.

The information provided on this form is used to verify your municipality's compliance with the minimum standards for administration and enforcement of the Uniform Code contained in Part 1203.

The information provided will also be used to develop education, outreach and support programs for municipalities and code enforcement officials.

Part 1203 reporting forms are updated and revised each year. Use only the form for the specific reporting year:

Part 1204 Review and Reporting Forms

The State of New York is responsible for the administration and enforcement of the State Uniform Fire Prevention and Building Code (the Uniform Code) and the State Energy Conservation Construction Code (the Energy Code) with respect to buildings, premises, and equipment in the custody of a State Agency.  Each State Agency is required to prepare an Annual Report of code enforcement activities relating to buildings, premises, and equipment in the custody of the Agency, and each State Agency is required to notify the Department of State when its Annual Report is complete.  See 19 NYCRR Section 1204.14. 

Note:  A State Agency satisfying the conditions set forth in 19 NYCRR Section 1204.15 is permitted to apply for an exemption from these Annual Report and notification obligations.

The Department of State has prepared a form that State Agencies can use (1) to prepare the Annual Reports required by 19 NYCRR section 1204.14.

Annual Report Form (Reporting Year: 2023) (Please note:  Each State Agency that has not received an exemption must prepare an Annual Report containing the information required by 19 NYCRR section 1204.14.  The Annual Report must be completed by February 1 of each year, and must include the information pertaining to the preceding calendar year. This Annual Report Form is provided as a courtesy of the Secretary of State of the State of New York to assist State agencies in fulfilling their Annual Report obligations.  Use of this Annual Report Form is optional; a State Agency may use this Annual Report Form or a form prepared by the State Agency and containing the information required by 19 NYCRR section 1204.14.).
