License History and Certification

License History:

Receive a sealed document, listing the complete history of how, when and where a license/registration was issued. Changes that were made to the license/registration will also be included.

Certification:

Receive a sealed document listing the current license/registration information including dates of term and if applicable, current place of business. Prior information such as changes made to the license/registration will not be included.

 

PLEASE NOTE: It is important that you indicate on the request form which state you are applying to for a license/registration, so you receive the required information. If you are applying to multiple states, there is a fee for each state you are requesting a license history/certification for.

The Division of Licensing Services maintains a six year file of licensees. Records prior to that time period are unavailable.
